All students are encouraged to submit your COVID-19 vaccination records to keep our campus safe and healthy! Our goal is to help increase the number of vaccinated people in our state, at GHC, and support student success. This will also allow GHC to better understand how many students and staff have been vaccinated to assist in the campus reopening planning.
A vaccination card or state vaccination record that shows either one dose of the Johnson and Johnson vaccine or two doses of the Moderna or Pfizer BioNTech vaccine.
To assist in the secure collection of vaccination record, we are partnering with MedProctor, a FERPA / HIPAA compliant verification website. To submit your record complete the following:
- Go to the MedProctor Website and create an account with your school email (ex. jdoe@my.ghc.edu). NOTE: You must use your GHC student email in order to create an account.
- For the “requirement group” please select “GHC Student”. Confirm that your Student ID is correct.
- Select “submit your health forms for free without the upgrade”. You DO NOT need to pay the $10 to submit your records.
- Upload your COVID-19 Vaccination Record or Request for Waiver form.

Request for Waiver
If you have a medical or religious reason to be exempt from receiving a COVID-19 vaccine, complete the Request for Waiver form found in the Med+Proctor system. Once waiver is completed, upload the document to the Med+Proctor site.
Any student, employee or visiting scholar who requests a waiver assumes the risk and responsibility for the consequences of not being immunized. The person must also acknowledge that additional health and safety measures may be required for them to participate in in-person activities and that they will abide by any directives from the State of Washington or the Grays Harbor Public Health in the case that they are identified as a close contact or test positive for COVID-19.
